National BSA policy requires all youth and adults attending an overnight activity to have a printed, signed copy of Parts A, B1, and B2 of the Annual Health & Medical Record (commonly called “Med Forms”) on file with the Pack.
Med Forms are valid for 12 months. A current form must be submitted for each family member:
- In some cases, you may be asked to submit them at a Pack meeting beforehand to help reduce paperwork during check-in.
- Turn in your forms to the Pack Medic or a leader at your first campout or overnight of the year.
